New York State Medical Treatment Guidelines for
Collateral Ligament Injury in workers compensation patients
The New York State workers compensation board has developed these guidelines to help physicians, podiatrists, and other healthcare professionals provide appropriate treatment for Collateral Ligament Injury.
These Workers Compensation Board guidelines are intended to assist healthcare professionals in making decisions regarding the appropriate level of care for their patients with ankle and foot disorders.
The guidelines are not a substitute for clinical judgement or professional experience. The ultimate decision regarding care must be made by the patient in consultation with his or her healthcare provider.
Description / Definition of Collateral Ligament Injury
Collateral Ligament Injury is rupture, sprain, or strain of the lateral or medial collateral ligament. A concurrent medial meniscus injury may also be linked to a medial collateral ligament damage.
Mechanism of Injury for Collateral Ligament Injury
Application of valgus or varus trauma force to the knee.
Specific Physical Findings of Collateral Ligament Injury
Tenderness over the medial or lateral collateral ligament, which rises with valgus or varus force given to the knee, and medial-lateral instability (knee should be evaluated in modest flexion).
Diagnostic Testing Procedures of MRI
Diagnostic Testing Procedures of and MRI are recommended in a small number of patients who may have Grade II or Grade III tears.
Non-Operative Treatment
Non-Operative Treatment is recommended in select patients as clinically indicated.
Rest/restricted activity, cane or crutch off-loading, ice, elevation, bracing, casting, orthotics, rehabilitation and active and/or passive therapy, NSAIDs, and therapeutic injections are among treatment options.
Surgical Indications / Operative Treatment
Surgical Indications / Operative Treatment are recommended in select patients.
Indications: An orthopaedic surgeon should be seen for a total Grade III collateral ligament damage.
